Emsella, and an honest account of what it does
Emsella uses high-intensity focused electromagnetic energy, shortened to HIFEM. You sit fully clothed on a chair while a focused electromagnetic field drives the pelvic floor into contractions stronger than voluntary effort can produce, because the field bypasses the conscious neuromuscular pathway. Thousands of contractions happen in a 28-minute session. The muscular response is the same as progressive resistance training anywhere else in the body.
The evidence is reasonable rather than overwhelming. A 2025 systematic review and meta-analysis in Neurourology and Urodynamics pooled seven studies and found a significant improvement in ICIQ-UI SF symptom scores against control, mean difference -3.03 (95% CI -3.27 to -2.79), with no safety concerns identified in most included studies (PMID 39760417). A proportion of the wider literature is manufacturer-funded, which is normal for device research but worth knowing.
NICE guidance NG123 recommends at least three months of supervised pelvic floor muscle training as first-line treatment for stress and mixed urinary incontinence. That is the starting point, and for some people it is the whole answer. Emsella earns its place where technique or adherence is the barrier, or as a way to consolidate what physiotherapy has already built.
It strengthens muscle. It will not repair structural damage to the sphincter mechanism, and it does not treat pelvic organ prolapse. If your symptoms point somewhere else, we will tell you at consultation and point you at the right pathway rather than selling you a course.
