NeoGen vs Fractional CO2 Laser: What the Downtime Trade Buys You
Nitrogen plasma is often sold as CO2 results with less recovery. The one high-energy study that tested it against that standard concluded the opposite. What each treatment does, and how to read the claim.

We do not own a fractional CO2 laser. We do offer NeoGen nitrogen plasma. Knowing that should sharpen how you read what follows, because the claim this article examines is one that would flatter us if we let it stand.
The claim is that nitrogen plasma gives you CO2-standard resurfacing with less recovery. It appears across the sector, in device marketing and clinic copy alike. It is not supported by the published work, and the single strongest study on high-energy nitrogen plasma says the reverse in plain language.
The mechanisms are not variations on a theme
A fractional CO2 laser emits light at 10,600nm, absorbed by water in the skin. That absorption vaporises tissue in narrow columns, drilling microscopic holes surrounded by a rim of thermal damage, and leaving untreated skin between the columns from which healing spreads. Tissue is removed. The controls are how deep the columns go and what proportion of the surface they cover.
NeoGen does not vaporise anything. Radiofrequency ionises nitrogen gas into plasma, which delivers heat to the skin surface across the entire treated field. Nothing is removed. The epidermis is thermally altered but stays where it is, acting as a biological dressing while regeneration happens beneath it, and separates days later.
So the honest structural comparison is: CO2 removes tissue fractionally, NeoGen heats tissue completely. One leaves untreated islands and a raw surface; the other treats everything and leaves a covered one. Those are different interventions, and ranking them on one scale of strength gets you the wrong answer.
What the evidence says about the comparison
The strongest high-energy nitrogen plasma study treated ten patients with acne scarring using a single session at 3.5 to 4.0 joules with two passes. Nine completed six months of follow-up. Patients rated their scarring 33% improved at six months, and physicians blinded to the timing rated the photographs 34% improved.
Those are real results and worth having. They are also, on the authors’ own assessment, less dramatic than what multipass ablative CO2 achieves. That is the paper generally cited to support nitrogen plasma for scarring, and it declines to claim parity with CO2.
No independent blinded head-to-head trial of nitrogen plasma against fractional CO2 has been published, so far as we can find. Anyone claiming equivalence is not citing one, because there is not one to cite.
What that leaves as the real trade
Fractional CO2 remains the more powerful resurfacing tool for established acne scarring and significant textural change. That is the honest position, and if that is your concern, a clinic offering CO2 is worth speaking to.
What nitrogen plasma offers instead is a different profile rather than a lesser version of the same one:
- Full-field treatment. No untreated columns between treated ones, which matters when the concern is uniform rather than discrete.
- The intact epidermis. The treated surface stays on as a dressing rather than leaving an open wound to care for, which changes the aftercare considerably.
- The eye area. Nitrogen plasma can be taken close to the lash line, which is a place most resurfacing is not comfortable going.
- No colour dependence. The energy does not need a chromophore, so it is not constrained by the colour of the target.
And what it does not offer is depth. If you need the depth, you need the depth.
Do not read “less downtime” as “little downtime”
The recovery difference is real but smaller than advertised. Ablative fractional CO2 typically involves a week or so of significant visible healing, sometimes longer depending on density and depth.
NeoGen’s published figures: at low energy, re-epithelialisation complete at four days and erythema averaging six, with the first session taking nine days to settle. At the high-energy settings that come closest to a resurfacing result, four to six days to re-epithelialise, with bronzing and peeling in between.
So the comparison is roughly several days against roughly a week and more, not several days against none. Any version of this treatment described as something you have in a lunch hour is being sold to you rather than explained.
